Audit defensibility in IDR: building a record you can stand behind

When a payer or a court looks back at a determination, the question is not 'did you win.' It is 'can you show your work.'

As of 2026-08-03 · 6 min read · Keyword: defensible IDR submission

Why defensibility is the operative word

Payers challenged the eligibility of roughly 42 percent of initiated disputes in the last six months of 2025, and QPA methodology is under active legal scrutiny after TMA III. In that environment, the record you can produce later is as important as the submission you make today.

What makes a record defensible

  • Source hashing: every document tied to a verifiable hash of its source.
  • Deterministic outputs: the same inputs produce the same evidence package.
  • Reviewer sign-off: a named human reviewer of record.
  • A complete, dated audit trail of every step.

Supports, does not replace, the reviewer

A defensible record exists to support your reviewer's judgment. Logic Refinery IDR-A preserves the trail; your team makes the call.
