Batching under the 2026 rules: the 50 line-item cap and what qualifies
Batching is where a lot of teams either save real time or get their disputes bounced. The 2026 cap changes the math.

What can be batched
The 2026 Final Rule limits batched determinations to 50 qualified IDR items and services. Batches generally must share a payer and involve the same or related items and services within the applicable window. The rule is designed to keep batches coherent enough for a single determination.
The viability math
With the administrative fee at $15 per party and certified IDR entity fees roughly $625 to $1,150 for batched determinations, the per-item economics improve as you batch coherent items up to the cap. But a mis-assembled batch that gets split or dismissed loses that advantage and the non-refundable fees with it.

Assembling a defensible batch
- 1Group by payer first, then by same/related codes.
- 2Keep each batch at or under the 50-item cap.
- 3Confirm every item is independently eligible.
- 4Preserve the batch's composition logic in the record.
